Thank you for choosing Shoppers Drug Mart #1377 for your COVID-19 vaccination. We carry three vaccines: PFIZER, MODERNA, and ASTRAZENECA Vaccines. Please read below before proceeding to book your appointment.

Please note that your appointment time is your check-in time. Time is still needed for paperwork that the team needs to do before you get your vaccine




ELIGIBILITY CRITERIA:


You must meet the eligibility criteria set by Province of Ontario for pharmacies. The eligibility criteria are subject to change. We will try to provide the most up to date eligibility criteria for pharmacies on this website however, Click Here to check the Government of Ontario Website for the most up to date eligibility.



PFIZER (Kids/Pediatric For 5 - 11 Years Old)


First dose:


You must be between 5-11 years old or turning five in 2021 (born in 2016).


Note that you cannot have any vaccine (including the flu vaccine) within 14 days (before or after) the pediatric Pfizer vaccine.


Second dose:


A minimum of 8 weeks (56 days) have passed since your first dose



PFIZER (Adult For 12+ Years Old)


First dose:


You must be born in 2009 or earlier


Second dose:

The second dose criteria depends on the vaccine you received for your first dose. You must meet one of the below eligibility criteria before proceeding to book your appointment. There can be no exceptions made.


First dose was Moderna: A minimum of 28 days have passed since your first dose


First dose was AstraZeneca: A minimum of 8 weeks (56 days) have passed since your first dose.


First dose was Pfizer: A minimum of 21 days have passed since your first dose


Third booster dose:


Only specific individuals are eligible to receive a 3rd booster dose. Please visit this website to check eligibility criteria.


In summary, the following groups can receive a booster shot at this time.


  • Immunocompromised: 56 days must have passed since your second dose and you will need a letter from your doctor indicating the criteria that you meet to be eligible to receive booster dose. If you are on an eligible immunosuppressant medication, a letter is not required but you must show proof that you are on an immunosuppressive medication. In specific situations for the immunocompromised a minimum interval of 28 days may be used but must be directed in writing by a health care provider.


  • Individuals over 18 years old: 84 days must have passed since your second dose.



MODERNA


First dose:

You must be 18+ year of age on the day of your vaccination.


Second dose:

The second dose criteria depends on the vaccine you received for your first dose. You must meet one of the below eligibility criteria before proceeding to book your appointment. There can be no exceptions made.


First dose was Moderna: A minimum of 28 days have passed since your first dose


First dose was AstraZeneca: A minimum of 8 weeks (56 days) have passed since your first dose.


First dose was Pfizer: A minimum of 21 days have passed since your first dose


Third booster dose:

ASTRAZENECA (NOT CURRENTLY AVAILABLE)


First dose:

No first doses are being given of the AstraZeneca Vaccine


Second dose:


Only if first dose was AstraZeneca and A minimum of 8 weeks (56 days) have passed since your first dose.



APPOINTMENT SLOTS:


All new appointment slots are released when a new shipment is received. If you do not see any open slots, check back frequently and add your name to the Vaccine Waitlist. You have up to 24 hours prior to your appointment to cancel/change your appointment. If you end up finding an earlier appointment time, please cancel your appointment so that it can be offered to another person. At this point, we are expecting one shipment a week. Please refrain from calling the store for stock updates or for vaccine appointment inquiries, as we have no visibility of any future shipments.

Please Fill all the field accurately, especially your e-mail (to receive your appointment confirmation) and contact information (so that we are able to get in touch with you).
